The Hudson Valley Asthma Coalition will be conducting the American Lung Association’s Asthma Educator Institute™  2-day course on November 14-15, 2019 | 8am-4:30pm to prepare qualified health professionals to sit for the National Asthma Educator Certification Board (NAECB) examination. The course will be at New York Medical College in Valhalla.

 

COURSE OBJECTIVES & AGENDA

The overall course objectives are:

  • To prepare participants to provide NHLBI and NAEPP guideline directed asthma care to patients, families and health care providers across all settings.
  • To identify the various roles of the asthma educator.
  • To help prepare participants to pass the National Asthma Educator Certification Board exam.
  • To network with health care providers/educators and organizations surrounding asthma care and asthma care policies.

The BH Community Crisis and Urgent Care Protocol improves access to coordinated evidence-based clinical treatment and supportive services that can enable individuals with BH disorders to address urgent and crisis BH issues safely in community settings with a variety of options. Blood Pressure Module Expand Continuing Education Credits:N/A
Intended Audience: Physicians, NPs, RNs, Medical Assistants or staff providing care to hypertension patients.

This module highlights best practices for capturing and documenting blood pressure derived from empirical evidence of missed opportunities. Controlling Blood Pressure is one of the ten priority performance improvement measures that MHVC has selected within the NYS DOH DSRIP program. The overarching goal of this measure is to reduce the number of Medicaid patients with high and uncontrolled blood pressure.

Blueprint for Health Equity Expand Continuing Education Credits:
Intended Audience:

The Blueprint for Health Equity is an experiential learning event, that will allow you to consider the work you already do or will do in the future, through the eyes of the people you will be serving. This workshop will explore the social determinants of health through a health equity and racial equity lens.

The day begins with a presentor, who will address the central question of how poverty and structural racism affect individuals, families and communities ability to achieve health equity. This presentor will discuss the structural causes of poverty, explore the intersection of structural racism and poverty as social determinants of health, and discuss racial health disparities.

Following this, participants will experience the Community Action Poverty Simulation (CAPS), designed by the Missouri Community Action Network. Participants will, with their assigned family, live out the role assigned to them for a total of four, fifteen minute weeks, allowing them to experience a full month living in poverty. This activity is followed by a debrief session in which participants reflect together on what they will take away from the experience.

The day concludes with a presentation from our keynote speaker, who will further explore the social determinants of health, racial equity, and the Results Based Accountability Model. The speaker will address how to put what you learn into action, and help clients and communities achieve health equity.

Length of training: Four (4) Hours

Heart failure, formerly known as congestive heart failure (CHF), means the heart can’t pump enough blood to meet the body’s needs. Participants will learn the common signs and symptoms of Heart Failure; selfmanagement techniques to prevent exacerbation of heart failure; and implications for case management of impacted individuals.

Hypertension is a serious medical condition where blood flows through the blood vessels with greater force than normal. This module will discuss hypertension, treatment and control and case management strategies to manage this serious medical condition.

The majority of cardiovascular disease (CVD) is caused by risk factors that can be controlled, treated or modified, such as diet, exercise, obesity, high blood pressure, cholesterol, smoking and stress. Culture Competency and Health Literacy Train-The-Trainer Expand Continuing Education Credits:
Intended Audience: Training Facilitators, Patient Care Managers/Supervisors, Health Educators /Coach

Length of Training: Four (4) hours.

Montefiore Hudson Valley Collaborative (MHVC) is offering train-the-trainer sessions on Cultural Competency & Health Literacy (CCHL). This training is MHVC’s commitment to “Enhance and transform health care of the population we serve in the Hudson Valley by committing to address the social determinants of health, that includes cultural and linguistic needs.

Depression Course Module Expand Continuing Education Credits:This module has been submitted for CE and CME credits and is pending approval.
Intended Audience: All Staff

This learning module is designed for primary care providers, care managers and behavioral health providers to increase knowledge and self-confidence in screening for and managing major depressive disorder using the PHQ9, measurement-based care and the collaborative care model.  The module uses a longitudinal series of annotated video case vignettes to demonstrate effective communication skills and the Collaborative Care Model in action.  A case-based post-test checks for understanding of covered content.

Specific content includes:

  • Details about the prevalence of major depressive disorder in the general population and in patients with comorbid chronic disease
  • How to screen for depression using the PHQ2 and PHQ9
  • How to have a conversation with a patient about a diagnosis of depression to
  • How to use shared decision tools to guide medication choice conversations, and counsel patients about medication side effects.
  • How to do a suicidal risk assessment using the P4 Screener, and how to manage a patient with active suicidality
  • How to use Brief Action Planning to support self-management of depression
  • How to collaborate with an extended care team  (primary care provider, care manager and psychiatrist) to implement the depression collaborative care model.
  • How to use measurement-based care to guide the management of depression by repeating the PHQ9 over time.
  • How asking about “what matters” can help support the development of care plans that meet patients where they are at.

By the conclusion of this activity learners will be able to:

  1. State three reasons why it is important to screen for depression in general medical settings
  2. Utilize the PHQ2 and PHQ9 to identify patients with depression in their practices and monitor treatment response
  3. State 4 questions of the P4 Screener to evaluate for active suicidality
  4. Utilize depression education, and self-management support strategies including BAP to manage patients with depression
  5. Understand the various roles of Collaborative Care team members and effectively collaborate with a behavioral health collaborative care team (Care manager, psychiatrist, PCP) to maximize care and make referrals as needed.

Montefiore Hudson Valley Collaborative (MHVC) is hosting Mental Health First Aid training in 2020. Mental Health First Aid is an 8-hour course that teaches you how to help someone who may be experiencing a mental health or substance use challenge. The training helps you identify, understand and respond to signs of addictions and mental illnesses.

About this course:

Just as CPR helps you assist an individual having a heart attack, Mental Health First Aid (Adult) helps you assist someone experiencing a mental health or substance use-related crisis. In the Mental Health First Aid course, you learn risk factors and warning signs for mental health and addiction concerns, strategies for how to help someone in both crisis and non-crisis situations, and where to turn for help.

When you take a course, you learn how to apply the Mental Health First Aid action plan in a variety of situations, including when someone is experiencing:

  • Panic attacks
  • Suicidal thoughts or behaviors
  • Nonsuicidal self-injury
  • Acute psychosis (e.g., hallucinations or delusions)
  • Overdose or withdrawal from alcohol or drug use
  • Reaction to a traumatic event

The opportunity to practice — through role plays, scenarios, and activities — makes it easier to apply these skills in a real-life situation.

Montefiore Hudson Valley Collaborative and Bronx Partners for Healthy Communities invite you to an informative and immersive program focused on the impact of poverty on our communities. Poverty has a direct link to poor health as evidenced in the Bronx where families live with significantly higher rates of chronic diseases like asthma, diabetes, and cardiovascular disease.

Experts will explore the relationship between poverty and health disparities and how we can improve our community members’ experiences when they visit us for services. Participants will take on the roles of family members living on a limited budget and learn about the challenges and difficult choices they make and the toll it takes on their health and access to care. Upon completing the simulation, participants will be prepared to identify opportunities to improve health equity for patients in their own workplace.

Intended Audience: Healthcare professionals in partner organizations looking to implement SBIRT in their practice and Mental health providers including social workers who provide direct service, supervision, and administrative oversight

Length of Training: Five and half (5.5) hours

Montefiore Hudson Valley Collaborative (MHVC) is introducing an instructor-led training on SBIRT. Screening, Brief intervention, and Referral to treatment (SBIRT). SBIRT is a comprehensive, integrated, public health approach that provides opportunities for early intervention before more severe consequences occur. Evidencebased tools that are demonstrated to be valid and reliable in identifying individuals with problem use or at risk for a Substance Use Disorder (SUD) must be used. This training is part of MHVC’s cross project training strategy to help build patient-centered self-management support skills and is beneficial to social work practice.

Trauma, Aging and the Implications for Treatment Expand Continuing Education Credits:2.0 Contact Hours
Intended Audience: Licensed Socials Workers, Licensed Mental Health Counselors, Psychoanalysts, Licensed Marriage and Family Therapists

The Geriatric Mental Wellness Alliance and The Trauma Institute of Orange County Presents Trauma, Aging and the Implications for Treatment

This presentation will focus on the impact of earlier trauma on the way individuals experience aging, illness, and helplessness as patients, as well as on the way the encounter with trauma victims impacts providers. The main learning objectives will be to allow providers to understand the interactions between trauma and aging, be aware of the individual variability in responses to trauma, and the often disguised presentation of Post Traumatic Stress Disorder (PTSD) in elderly trauma survivors.

The lecture will describe the pervasive reality of dehumanization in modern medical settings, its functional and non-functional causes, and factors that place elderly trauma survivors at an elevated risk for such treatment. Finally, some of the ways to counter these implicit processes will be addressed in order to prevent traumatization and re-traumatization of individuals.

Value Based Payment (VBP) Boot Camps Expand Continuing Education Credits:
Intended Audience:

VBP bootcamps are a regional learning series created by the Department of Health with a goal to equip VBP contractors and interested parties such as Managed Care Organizations, Providers, Associations, and Community Based Organizations, with the knowledge necessary to implement payment reform.

The VBP bootcamps will be one, day-long session in five regions across the State. The VBP bootcamp is tentatively scheduled for 9:00AM-5:00PM. The same subjects and content will be covered in every region.

The bootcamps will consist of breakout sessions and workshops on the following subjects:

  1. VBP Financing
  2. VBP Contracting
  3. VBP and the Social Determinants of Health/Community Based Organizations
  4. VBP Quality Measures
  5. VBP for Managed Long Term Care

**All of the sessions for the Capital Region VBP bootcamp will be webcast live and recorded. The link will be sent out when it becomes available.

Registration for each region will open approximately 3 weeks in advance of the session and close 1 week before the event. The registration link will be posted when it becomes available.

 These events are free and open to all interested parties. Space will be limited for each region, and space will be on a first come, first serve basis. We ask that all organizations limit participation to no more than 4 participants.

As a reminder, VBP bootcamps are being considered the ‘4th semester’ of VBP University. If you attend a VBP bootcamp, and have completed all 3 semester quizzes of VBP University with a passing grade of 80% or higher, you will receive a printed copy of your ‘VBP University diploma’ at the session you attend. Value Based Payments (VBP) – Contracting with Managed Care Organizations (MCOs) Expand Continuing Education Credits:N/A
Intended Audience: middle or upper management whose responsibility is to directly engage in contracting and/or manage relationships with payors.

As health care transitions into a value-based environment, Montefiore Hudson Valley Collaborative (MHVC) is offering training to partners on the key components and players of MCO contracting, to prepare partners to be successful in this environment.

During this session attendees will explore key foundational concepts of contracting with managed care organizations (MCOs).By the end of this session participants will be able to:

• Identify key stakeholders and their respective responsibilities within managed care contracting
• Understand the various types of managed care contracts and services, including reimbursement models as well as what goes into an MCO contract
• Recognize functions covered under managed care contracts
• Measure and monitor success in MCO contracting through various metrics
• Understand how the process differs for the various provider groups (e.g.BH, CBOs, PCPs, ACOs)
