The symposium will provide a forum to share the perspectives and insight of the DSRIP Performing Providers Systems, Physical and Behavioral Health Providers, Managed Care Organizations, and community organizations addressing the Social Determinants of Health. The goal of the summit is to provide a platform for these perspectives on advancing VBP and gain understanding for further discussion.

The NY Medicaid Population Health Symposium will occur across two days (November 18-19, 2019) and the program will include plenary sessions, best practice sessions, panels including multiple perspectives and experiences, and poster presentation and viewing. A detailed agenda will be provided soon.
