Montefiore Hudson Valley Collaborative (MHVC) is offering train-the-trainer sessions on Care Management for Health Homes in 2017. This full day workshop is designed to activate health home care management trainers to engage their care management staff in promoting positive patient outcomes while contributing to healthcare transformation.
