Learners who complete this module will receive elements of effective integration and examples of tools health care organizations utilizes in creating an integrated delivery system.

This module will provide information on members of an extended care team and how they contribute to population health. The learner will also receive elements of patient-centered care and how they promote population health.

This module will explain the context for population health through defining “population health” and differentiating it from “public health” and “community health”.

At the end of this module, the learner will be able to describe the different health care settings that care for certain conditions and the role of a medical neighborhood. This module will also define characteristics of an integrated delivery system.

This online training provides a foundational learning on DSRIP, the role of Performing Provider Systems, and the role of network partners as part of MHVC.
